Can you determine whether a medical record was changed?
An audit trail and related metadata may help establish when a record was accessed, amended, signed, or corrected. The available information varies by EHR system and by what has been preserved or produced. We do not promise to prove fraud or record tampering.
What is an EHR audit trail?
An Electronic Health Record (EHR) audit trail is a machine-generated log that records every access, amendment, signature, print, or edit event associated with a patient's digital chart, including timestamp and authorship data.
What documents are needed?
We typically require complete sets of the medical records in question, billing/coding histories, provider notes, credentialing files, and when available, full EHR audit trails or system metadata logs.
How much does an initial review cost?
Initial individual engagements generally begin with a $2,500 advance retainer, billed against professional time at $350 per hour. The necessary time depends on the volume, organization, and complexity of the records. Hive will obtain authorization before materially expanding the scope.
Can you testify?
Yes, but acceptance of a consulting matter does not automatically constitute agreement to provide testimony. Formal expert witness reporting, deposition, and trial testimony are subject to separate screening and a distinct written agreement.
Is every case accepted?
No. All inquiries undergo a strict suitability and conflict review. We only accept matters where there is a clear analytical assignment, sufficient documentation, and no conflicts of interest.
How are sensitive records transferred?
Sensitive records are transferred exclusively via secure, individual client upload links provided after a conflict check is cleared and an engagement agreement is signed. We do not accept file uploads on public forms.
Can you work with my attorney?
Yes. Hive Advisory frequently coordinates with and supports retained legal counsel to prepare chronologies, analyze record metadata, and support expert witness strategies.
